What is an Advanced Care Directive?

An Advanced Care Directive (ACD) is a legal document that allows you to specify your wishes regarding your end of life decisions, in the event that you no longer have the capacity to make those decisions yourself.

An ACD can be included within your Appointment of Enduring Guardian or can be a stand-along document. Regardless, an ACD is an important part of your Estate Plan. Without one, choices about your care can be made without your imput.
